Filling this out before you come saves time at check-in. It takes about three to five minutes.
Have these ready: your insurance card, a list of the medications and supplements you take, and — if you are being treated for an auto accident or a work injury — your claim number and your adjuster's contact information.
Prefer paper? That's fine — arrive a few minutes early and fill it out at the clinic.

What to expect at your first visit?
The first visit begins with a full history: what brought you in, and also your sleep, digestion, energy, and how the problem behaves over the course of a day. I examine the tongue and pulse, and the affected area itself where that applies. You then rest on the treatment table with the needles in place for about forty minutes. Plan on roughly an hour and a half for a first visit, and wear loose clothing if you can.
